# Health Coverage Exemptions / Responsibility Payment - Form 8965

**Form 8965 - Health Coverage Exemptions/ Responsibility Payment** 

 

Note: The Shared Responsibility Payment and Form 8965 will no longer be available from the tax year 2019 (Filing in 2020). The information below is for those tax years before 2019. 

 

To access Form 8965: 

- Go to the Main Menu of the tax return (Form 1040)
- From the menu, select Other Taxes Menu
- Proceed to Health Coverage Exemptions / Responsibility Payment - Form 8965
